Desethylamiodarone(CAT: I025927) is the primary active metabolite of amiodarone, a widely used Class III antiarrhythmic drug. It is formed through the N-deethylation of amiodarone in the liver. Like its parent compound, desethylamiodarone inhibits multiple ion channels, including potassium (K⁺), sodium (Na⁺), and calcium (Ca²⁺) channels, and blocks β-adrenergic receptors, contributing to its antiarrhythmic effects. This metabolite is particularly significant in prolonging the action potential duration and refractory period, making it effective in managing ventricular and supraventricular arrhythmias. Desethylamiodarone is also associated with some of amiodarone’s long-term toxicities, such as pulmonary and thyroid dysfunction, highlighting its importance in cardiovascular research.
