Melarsoprol is an arsenical drug used in the treatment of African trypanosomiasis, or sleeping sickness, caused by Trypanosoma brucei. It exists as a mixture of diastereomers, with each isomer having distinct pharmacological properties. Despite its efficacy, melarsoprol treatment is associated with severe side effects, including neurotoxicity, which limits its use to late-stage infections when no other treatments are available.
