N-Desmethyl sildenafil is an active metabolite of sildenafil, the active ingredient in the medication Viagra, which is commonly used to treat erectile dysfunction and pulmonary arterial hypertension. This metabolite is formed when sildenafil is metabolized in the liver, primarily by the enzyme CYP3A4. N-Desmethyl sildenafil retains much of the parent compound’s ability to inhibit phosphodiesterase type 5 (PDE5), which enhances the effect of nitric oxide in the body, leading to vasodilation and improved blood flow. Although it is less potent than sildenafil, it still contributes to the overall pharmacological activity and duration of action of the drug.
